No. Tick tattoo and the finished form is in your account when you land on the dashboard. Editing it is optional.
Can I keep my own consent wording?
Yes. Paste it in and it becomes your form. Past submissions keep the exact wording that was signed.
Does it handle walk-ins?
Put the QR code on the counter. They fill it in while you set up, and it is waiting for you when you sit down.
How long are records kept?
